New Patient Registration Process

Please read all of the steps before submitting an Appointment Request Form.

Step 1: Let’s Begin
We are thrilled that you're ready to start your journey with us! Begin by completing our New Patient Appointment Request Form. This is your chance to tell us about your needs, whether for yourself or your child. Rest assured, your information will be handled with the utmost care and confidentiality. Please note that Richard Akers is not accepting new patients at this time.

Step 2: Our Team Reviews New Patient Inquiries
Once your form is submitted, our mental health behavioral health team will carefully review it and respond with any additional questions. We aim to address all New Patient Appointment Requests within 5-7 business days, although this may vary based on the volume of inquiries. If we determine that we are a good fit, we will contact you to guide you through the next steps of our registration process

Step 3: Complete Your New Patient Registration Forms
As part of the registration process, you are required to complete a New Patient Registration Packet through our patient portal called MIYO. It's essential that MIYO is active and all forms are filled out before scheduling an appointment. Upon logging into MIYO, you can locate these forms under "Clinical Forms." The packet includes a patient health history form, payment and insurance authorization forms, practice policies and agreements, and measurements.

Step 4: Stay Connected with Spruce
We use Spruce Health, a secure HIPAA-compliant messaging platform, to communicate with our patients. For easy access and streamlined communication with our practice, we recommend downloading the Spruce app on your mobile device. Step 6: Schedule Your Evaluation
After we have reviewed your completed new patient registration paperwork, we'll be ready to book your first appointment! We will contact you via text message through the Spruce Health app to find a time that suits your schedule and our earliest availability. Please note that appointments are only offered to patients who have completed all the necessary steps.

Scheduling Appointments: New patients, whether adults or minors are usually scheduled within 2-4 weeks after all necessary paperwork is completed and payment/insurance details are verified. Please do not use the portal to schedule new patient appointments, as these appointments will be denied. The portal is reserved for current patients who have already completed their initial evaluation.

Struggling to Access Your Forms?

1) To create a portal account, whether you're a new or existing patient, begin by visiting the practice portal URL. From there, access the "Account Setup on Your Desktop Browser" link provided in your invitation email. Follow the prompts to select "Create Account" and enter the access code found in your MYIO email or text invitation. If you haven't received an invite, reach out to your practice for assistance. Remember that the access code expires after seven days.

2) Complete the account setup by entering your portal email address and proceeding to click "Verify My Email". Enter the verification code sent to your email and confirm it to finalize your email verification. You can resend the code if necessary before the countdown expires. Your portal email will serve for MYIO password resets and to receive updates, messages, or shared documents.

3) For new patient onboarding, provide your personal details, ensuring to fill in required fields marked with *. These details may vary based on practitioner settings, and you can manage and update your patient record in the "My Account" section of MYIO.

  • Required personal details include your First Name, Middle Initial, Last Name, Suffix, Preferred Name, Preferred Pronouns, Gender Identity, and Sex as per your insurance policy. Create your login credentials by selecting a username and setting a password that meets security criteria (eight or more characters, including at least one uppercase letter, one lowercase letter, and one number or special character). Confirm your password and proceed by clicking "Next".

4) Once logged into the portal, locate your new patient forms under "Clinical Forms". Please complete all new patient forms. Once new patient forms are completed we will reach out to schedule you at our soonest available time.
