Patient Rights & Responsibilities

The health and well-being of patients depend on a collaborative effort between patient and physician in a mutually respectful alliance. Patients contribute to this alliance when they fulfill the responsibilities they have, seek care, and be candid with their physicians. Physicians can best contribute to a mutually respectful alliance with patients by serving as their patients’ advocates and by respecting patients’ rights. These include the right:

  • To have courtesy, respect, dignity, and timely, responsive attention to his or her needs.

  • Receive all communications in a language and/or manner you understand/

  • To receive information from their physicians and to have the opportunity to discuss the benefits, risks, and costs of appropriate treatment alternatives, including the risks, benefits, and costs of forgoing treatment. Patients should be able to expect that their physicians will provide guidance about what they consider the optimal course of action for the patient based on the physician’s objective professional judgment.

  • To ask questions about their health status or recommended treatment when they do not fully understand what has been described and to have their questions answered.

  • To make decisions about the care the physician recommends and to have those decisions respected. A patient who has decision-making capacity may accept or refuse any recommended medical intervention. To have the physician and other staff respect the patient’s privacy and confidentiality.

  • Receive information about your diagnosis, treatment, and expected result from your provider or designated staff in terms you can understand. When it is medically inadvisable to provide such information to a patient, the information is provided to a person designated by the patient or to a legally authorized person.

  • Exercise your rights without being subjected to discrimination or reprisal. Personal privacy and confidentiality concerning your medical care. Information can only be released with your consent, except as provided by law. You have the right to be advised as to the reason for the presence of any individual. HIPAA regulations will be observed.

  • Within the confines of the law, review your medical records. All communications and records pertaining to your care will be treated as confidential.

  • To obtain a second opinion.

  • To be advised of any conflicts of interest their physician may have with respect to their care. To continuity of care. Patients should be able to expect that their physician will cooperate in coordinating medically indicated care with other healthcare professionals and that the physician will not discontinue treating them when further treatment is medically indicated without giving them sufficient notice and reasonable assistance in making alternative arrangements for care.

Patient Responsibilities

  • Complete and accurate information about yourself, including your full name, address, home telephone number, date of birth, and insurance information, is required. You must also provide photo identification and your insurance card. Along with this, you are expected to divulge your health and medical history, including your present medical condition, past medical history, medicines, vitamins, herbal products, and any other health-related matters.

  • It is your responsibility to inquire if you do not comprehend the explanation of your diagnosis, treatment, prognosis, or any instructions. Instructions concerning medications, follow-up visits, patient education, and notifying your health care provider if your treatment plan cannot be followed must be followed. If you do not follow the care and treatment plan, you are responsible for the outcomes.

  • If you need to cancel an appointment, please notify Canyon Behavioral Health in advance, and you must arrive as scheduled for appointments. You will be responsible for any charges that are billed to you as a result of receiving services at Canyon Behavioral Health.

  • Without their permission, please refrain from videotaping, photographing, or otherwise recording Canyon Behavioral Health health care providers, employees, or other patients. Healthcare providers and Canyon Behavioral Health employees have the right to prohibit photography or videotaping at any time they believe it creates interference or is inappropriate.
