Good Faith Estimate
Starting from January 1, 2022, individuals who are uninsured or self-paying (i.e., those whose claims are not submitted to their health plan) are entitled to receive a Good Faith Estimate of the expected total cost of any healthcare items or services that are scheduled, or upon request, prior to the delivery of those items or services.
Getting a "Good Faith Estimate" means receiving a reliable estimation of the total expenses you can expect for your healthcare needs. This should cover the expected charges for the main item or service you will receive, as well as any costs related to other items or services that may be provided along with it, such as medical tests, equipment, prescription drugs, and hospital fees.
If you schedule a health care item or service with Canyon Behavioral Health at least 3 business days in advance, Canyon Behavioral Health must provide you with a Good Faith Estimate in writing within 1 business day after scheduling.
If you schedule a health care service or service with Canyon Behavioral Health at least 10 business days in advance, Canyon Behavioral Health must provide you with a Good Faith Estimate in writing within 3 business days after scheduling.
If you ask Canyon Behavioral Health for a Good Faith Estimate (before you schedule a service with Canyon Behavioral Health), Canyon Behavioral Health must provide you with a Good Faith Estimate in writing within 3 business days after your request.
Make sure you save a copy of your Good Faith Estimate and the bill you receive from Canyon Behavioral Health
If you receive a bill that is at least $400 above the Good Faith Estimate provided by Canyon Behavioral Health, federal law allows you to dispute the bill.
